Rapid response hospital discharge and admissions avoidance services
Working in partnership with Anglian Community Enterprise and the North East Essex Clinical Commissioning Group, we have developed an innovative and responsive service to assist with meeting the increasing demands on local hospitals.
Home from hospital
Our Hospital Discharge Service helps to ensure hospital discharges are carried out rapidly and safely to avoid longer than necessary hospital stays.
Our committed team of Care and Support Assistants help customers with plaster of paris or a body brace in situ with activities such as washing, dressing and meal preparation for up to 12 weeks so that they can recover in their own home.
Admissions avoidance
Our Admittance Avoidance service enables people who need care following an accident or episode of ill health to be cared for at home for six days or 11 visits. This is instead of being admitted to hospital, vitally reducing the strain on local services.
These services are available 7 days a week and operate between 7am and 10pm.
Night owls
Our dedicated team of Night Owl Care and Support Assistants travel through the night providing reassurance, care and support to those who have been assessed as needing a night visiting service.
The Night Owl Service operates between the hours of 11pm and 7am and is available seven days a week, 365 days a year.
Under all these services, our committed team of Care and Support Assistants help customers with:
- meal preparation
- dressing and undressing
- rising and retiring
- personal care including bathing, shaving, showering, washing hair
- assistance with intimate personal care
- oral health care
- catheter care
- nutrition and hydration needs
- incontinence management
- assistance with surgical stockings
- medication management
- maintaining your home
- home safety checks
- fitting of immediate equipment to prevent trips or falls
- moving of light furniture
- fitting a key Safe
- companionship
- signposting
- shopping
Our Home from Hospital and Admission Avoidance Service is available to customers who are registered with a North East Essex GP and are over the age of 18 years.
The service is available in North East Essex only (Colchester and Tendring local authority areas with a GP registered with NEE CCG).
